This notice describes how information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our duties

We are required by law to protect the privacy of your health information, to give you this notice, and to follow the terms of the notice currently in effect.

How we may use and disclose your information

• For your care. To provide, coordinate, and manage the services in your service plan, including sharing information with your caregivers and, where appropriate, other providers.
• For payment. To bill and be paid by Medicaid, a managed care entity, or you.
• For our operations. Quality review, training, supervision, and licensure.
• When the law requires it. Including reporting suspected abuse, neglect, or exploitation, and responding to the Indiana Department of Health.

Other uses and disclosures require your written authorization, which you may revoke at any time in writing.

Your rights

• Inspect and get a copy of your record
• Ask us to correct it
• Ask for a list of certain disclosures we have made
• Ask us to limit what we use or disclose
• Ask us to contact you a particular way or at a particular address
• Get a paper copy of this notice at any time

Complaints

If you believe your privacy rights have been violated, contact us at 317-688-8233. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
