This policy explains what Absolute Choice Home Care LLC collects through absolutechoicehc.com, why, and what we do with it. Health information we hold as a care provider is covered separately by our HIPAA notice.

What we collect

Through the forms on this site we collect the name, phone number, email address, location, and whatever details you choose to type into the message field. We record whether you checked each consent box and when. We also collect ordinary web analytics — pages viewed, approximate region, device type — which are not tied to your name.

Text messages

If you check the box giving permission, we may send you text messages. There are two separate permissions and they are never bundled together:

• Service messages about your request, your schedule, your care, or your job application.
• Marketing messages — announcements, programs, events, and referral or hiring bonuses. This is optional. Consent to marketing messages is never a condition of receiving care or of employment.

Message and data rates may apply. Message frequency varies; marketing messages are limited to about four per month. Reply STOP at any time to opt out, or HELP for help. You can opt out of marketing messages and continue receiving service messages. Opt-outs are honored immediately and permanently.

We do not sell, rent, or share your phone number. Mobile information is never shared with third parties or affiliates for marketing or promotional purposes. Phone numbers are used only by our own staff to contact you.

Who sees your information

Our staff, and the service providers we use to run the agency — our customer records system, our phone and messaging provider, and our email provider. They act on our instructions. We do not sell your information to anyone.

We may disclose information when the law requires it, including to the Indiana Department of Health in connection with licensure or a complaint investigation.

How long we keep it

Client and employment records are kept as long as Indiana law requires. Website enquiries that do not become client or employment records are kept up to three years.

Your choices

You can ask us what we hold about you, ask us to correct it, ask us to stop contacting you, or ask us to delete an enquiry record. Call 317-688-8233 or email info@absolutechoicehc.com.

Children

This site is not directed to children and we do not knowingly collect information from anyone under 13.
